Lasallian Formation and Social Action
The Lasallian Formation and Social Action Office, as an institutional department, functions within the Office of the Vice Chancellor for Lasallian Mission and Linkages, collaborating to bring to life the Vision-Mission, Identity, and Values of De La Salle Medical and Health Sciences Institute.
Our shared mission, in collaboration with various Lasallian Mission and Linkages units, is to provide spiritual and pastoral opportunities to all members of our community - students, faculty, staff, non-teaching, alumni, and agency-hired personnel regardless of their own viewpoints and religious affiliations. We give space to every member to enable them to dwell in the presence of God, guided by the spirituality of St. John Baptist de la Salle and embodying the core values of our Institution – the spirit of faith, zeal for service, communion in mission, and reverence for life.
OBJECTIVES
The Office of the Lasallian Formation and Social Action strives to foster Lasallian Spirituality across all sectors, including students, adults (faculty, staff, non-teaching personnel, alumni, and agency-hired]) through the facilitation of engaging and relevant Lasallian Formation Modules, creatively lived out by active participation and volunteering in the social action and community development programs. The LFSA Office oversees the formation of Students, Adults, and Special Groups, as well as community-based and outreach initiatives, advocacy, and volunteer programs. Institutional outreach programs are centralized in the LFSA Office, handling database management and allocating budgets to requesting colleges and departments. The LFSA Office oversees the Lasallian Formation and Social Action Facebook Page, utilizing it to promote Lasallian Formation and Social Action initiatives in the DLSMHSI Community.
INSTITUTIONAL LASALLIAN FORMATION (ILF)
Adult Formation
● Lasallian Modules for Adults: Module 1-20
● New Formation Modules - VITAL Program (Induction, Belonging, Commitment, Co-Responsibility, Wisdom)
Student Formation
● Lasallian Modules tailored for students
(for Undergraduate 2nd Year and 3rd Year Students)
Specialized Formation
● Lasallian Modules for special groups, including the Student Transformative Accompaniment and Renewal (STAR) Formation, Administrators Lasallian Leadership Formation Program, Reflect and Recharge (R&R) with Bro for administrators, medical interns/residents/consultants, alumni, athletes, performers and coaches, agency-hired teachers.
INSTITUTIONAL SOCIAL ACTION (ISA)
Community-based and outreach, such as:
● Medical-Dental Mission
● Operasyon-Tuli
● Food Service for the Community
● Blood Letting Activity
● Calls for Donations
● Relief Operations
● Activities with Community Partners such as Bahay Pag-asa (Dasmariñas), Bahay-Tuluyan (Dasmariñas), Kuya Center (Quezon City), Burol Main Lorenzo (Siko) Community, other Kalinga sa Barangay programs, and coordinating all OVCLML-ILMC approved outreach programs
Lasallian Advocacy
Lasallian Justice and Peace Commission (LJPC) advocacies, focusing on themes such as:
● Good Governance and Peace
● Poverty Alleviation
● Youth-at-Risk
● Environment with special emphasis on Disaster Risk Reduction and Management (DRRM)

● One Million Trees and Beyond (OMTB)
● TANIM at ANImo
● Season of Creation
● International Lasallian days for Peace
● Coastal Clean-Up
● Lasallian Guadalupe Project (Advocacy for the Unborn)
● Karitong Lasalyano (Advocacy for the Poor)
● Kalinisan Project
(Advocacy for Work Spirituality of the Agency-Hired)
Lasallian Volunteer (Lingkod Lasalyano)
● Involves student and adult volunteers who actively participate in formation sessions, and engage in Lasallian Mission-Social Action programs
● Contributes to managing social media channels to amplify the message of service and advocacy of the DLSMHSI
● Collaborates on initiatives such as “Hair For You” in partnership with Makati Med Cancer Center and the Diocese of Imus Catholic Educational System, Inc. (DICES).

INSTITUTIONAL COMMITTEES
Institutional Lasallian Mission Committee (ILMC)
Chaired by the LFSA Director, the ILMC is a formal institutional group that serves as the representative body for the entire DLSMHSI Community. The ultimate goal is to revitalize and sustain the Lasallian mission (formation, spirituality, social action, and culture) through collaborative and unified efforts across various departments and units, where we can share ideas and resources to create meaningful and long-term mission projects while strengthening the bonds within the Lasallian community.
Institutional Lasallian Formation Committee (ILFC)
Chaired by the LFSA Director, the ILFC is a formal institutional group comprising volunteer formators. Their primary responsibility is to develop the new Lasallian Formation Modules known as VITAL. Additionally, the ILFC oversees the seamless implementation of all formation modules across the entire DLSMHSI Community.
The new Lasallian Formation module is based on the book “Lasallian Formation for Mission: The Pilgrim’s Handbook” (2019).

VITAL is an acronym representing the five (5) thresholds, which stands for:
V-alue-based Induction (Paghasik)
I-nclusive Belonging (Pagsibol)
T-otal Commitment (Pagbunga)
A-nimating Co-Responsibility (Pag-ani)
L-ife-saving Wisdom
(Pagsaksi)
